Skin Cancer Screening Clinic May 6
The LRMC Dermatology Clinic is opening its doors for a skin cancer screening clinic May 6. Patients who have had a skin cancer screening within the past 12 months are ineligible, and no other issues will be addressed or treated during this appointment.

A limited number of appointments are available. Appointments can be made by calling DSN 486-5762 or Commercial: 06371-86-5762.

Sleep Study
Do you have problems sleeping? If you are 18 years or older, a military beneficiary (service member, retiree, family member), or have had problems sleeping for 6 months or longer, you may be able to take part in a study using a microcurrent therapy for insomnia.

Participation is voluntary and confidential. Please call DSN 590-5641, Commercial 06371-9464-5641 or e-mail sleep.study@yahoo.com for more information.

TBI Self-Referral Call Line Now Available
The ERMC Active Duty TBI Self-Referral Call Line is now available. Any active duty service member who feels they may have signs and symptoms of a TBI, please call DSN 590-4979 or Commercial 06371-9464-4979 for a brief screen and referral for appointment with a TBI Provider.

New Location for Outpatient TBI Screeing Begins Ap
Beginning April 15, Outpatient TBI Screening will be done at the Traumatic Brain Injury (TBI) Program (Bldg. 3737) near the Fire Station and across the street from the MTD. The entrance is at the back of the building (not facing the street). Please call DSN 590-4979 or Commercial 06371-9464-4979 to set up a time to be seen by the Inpatient Screener or for more information.

Free Tax Assistance at LRMC
Free tax assistance is available from 9 a.m. - 4 p.m. Mondays thorugh Fridays at the Landstuhl Tax Assistance Center located in Room 108B in Bldg. Bldg 3701 (next to launderette). For more info call DSN 486-6334 or Commercial 06371-86-6334.

How do I get my birth certificate?

It's commonly misperceived that the Army hospitals issue birth certificates. We do issue a report of live birth that the U.S. Consulate uses to issue a Report of Live Birth, or FS-240.

The FS-240 is valid in all U.S. States and Territories as the equivalent of a birth certificate. To obtain a copy, follow the instructions at the U.S. Department of State at http://www.travel.state.gov/law/info/overseas/overseas_703.html

The American consulate is the only agency abroad that can issue an American Birth Certificate and Passport. However, your Regional Personnel Center can assist and must collect, verify and forward all requests to the American Consulate.

The American Consulate does not accept walk-ins. You must utilize your personnel canter. For more information, please contact your regional center or call Landstuhl Regional Medical Center Birth Registration Office, at DSN 314-486-6115 or Commercial 011-49-6371-86-6115.

Where can I find my medical records?

Your Outpatient Medical Record should be kept at the Medical Center where your Primary Care Manager (PCM) is located. If your PCM is located in LRMC, then your Outpatient medical record should be maintained at the LRMC outpatient records room.

For more information on locating your medical records or starting a medical record, call LRMC Outpatient Records Room at DSN 314-486-8047/8475 or Commercial 011-49-6371-86-8047/8475.

For information on your Inpatient Medical Records, if you were ever admitted into a Medical Treatment Facility (MTF), then call the MTF you were admitted to. They will give you copies of your inpatient medical records. For LRMC patients, call DSN 314-486-8822 or Commercial 011-49-6371-86-8822.

If the records have been retired, visit the National Archives in St. Louis at http://www.archives.gov/st-louis/military-personnel/other-medical-records.html for further assistance.

Below are instructions for the three options for making a request to the LRMC Inpatient Medical Records Office:
 
1.    Send an Encrypted E-Mail with a completed DD Form 2870 (blank form attached with this e-mail) and a copy front & back of the requestors ID (it can’t be a DOD ID Card) to LRMCPADCorrespondenceRequest@amedd.army.mil. The form can also be downloaded from search.usa.gov at http://www.tricare.mil/tma/MMSO/downloads/dd2870.pdf

2.    Mail a completed DD Form 2870 and a copy  front & back of the requestors ID (it can’t be  a DOD ID Card) to:

Upon receipt of any of the above three we will send via email the acknowledgement and begin the processing. Normal processing at this time is 4-6 weeks. Sensitive information can take 4 weeks longer.

How do I get a Death certificate?

The Death Certificate is issued to the next of kin of a bereaved member immediately after the death of a family member by the MTF where the death occurred. For example, when an American is pronounced dead by a physician in Landstuhl Regional Medical Center, the hospital will immediately prepare a death certificate signed by the pronouncing physician. A certified copy, usually about 10 to 15 certified copies are given to the next of kin. When death occurs in a local German hospital, the German hospital gives LRMC a German version of the certificate which is then translated into an American death certificate by LRMC. The next of kin have a choice of which death certificate they want.
For burial here in Germany, they will need a German death certificate, LRMC casualty office will assist in getting one. For burial in the United States, the next of kin only need the American death certificate. How important is DEERS Registration for newborns?

DEERS registration is very important for all members of the military, their family members, civilian DOD employee and their family members as well. Newborns should be enrolled in DEERS within 7 days of the child's birth. If your child is not enrolled in DEERS before your child's first well baby appointment, you could receive a bill from the hospital treasury office for indebtedness. Also, TRICARE would not be able to assign your child a Primary Care Manager (PCM) if your child is not enrolled in DEERS.

Appointments are easier, quicker and hassle free to make when a child is assigned a PCM. Save yourself some trouble, report your child's birth to the nearest ID card/DEERS section in your community. How can I find a Civilian Medical Job?

All medical positions are listed on a centralized site at the U.S. Army Civilian Personnel Online Web site under a separate listing for Medical Positions at http://cpol.army.mil/

You can also apply for a position at Landstuhl Regional Medical Center by sending a resume to lrmc.jobs@amedd.army.mil.

From there, the resume is screened by a recruiting specialist who will contact applicants if they are qualified and a vacancy for that position needs filling.
